Contractors and subcontractors must provide the Public Works Department with this necessary information to receive addendums, information regarding the project, and to be listed on the mandatory Plan Holder's list. If you are a subcontractor that intends to provide services/costs to a prime contractor, you must also fill out the Plan Holder’s form
